Position Description
POSITION TITLE: Case Manager
Department: Case Management
Reports
To: Director of Case Management
SUMMARY: Effectively uses all appropriate resources to
help students resolve personal, emotional, and social problems that interfere
with their adjustment to school and their capacity to enjoy the fullest
benefits of the education offered.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Other duties may be assigned.
Þ
Performs casework service with individual
students to correct those personal, social, or emotional maladjustment's
related to their educational and social progress.
Þ
Performs casework service with parents as
an integral part of the task of helping students, to increase the parents'
understanding, their constructive participation in resolving their child's
problems, and their knowledge and use of appropriate resources available in the
community.
Þ
Establishes and maintains effective
liaison with the various offices and agencies within the community that may
provide specialized or professional help to students and their parents, and
serves as the referral agent to those offices and agencies.
Þ
Consults and collaborates with school
personnel and social agency personnel in gathering and giving information on a
case, and in establishing and planning for respective roles in helping to
re-orient and motivate students.
Þ
Prepares and maintains an inventory of
services offered by community health and welfare agencies.
Þ
Makes home visits for the purpose of
gathering helpful information on a students' background.
Þ
Serves as a liaison between home, school,
and social agencies when considerable follow-up is necessary.
Þ
Assists students directly toward
adjustment to school.
Þ
Keeps sufficient records of cases for use
by school staff members and outside agencies.

EDUCATION
and/or EXPERIENCE:
Associate or Bachelor Degree in Psychology or
Social Work, Human Services or related field.
One year successful work experience in human services. Must be able to
effectively deal with the public, staff, students and parents both in person
and on the phone.
LANGUAGE
SKILLS:
Ability to read and interpret documents such as
safety rules, operating and maintenance instructions and procedure manuals.
Ability to write routine reports and correspondence. Ability to present information in one-on-one
and small group situations to customers, clients, other employees and/or
students.
MATHEMATICAL
SKILLS:
Ability to apply concepts such as fractions,
percentages, ratios, and proportions to practical situations.
REASONING
ABILITY:
Ability to deal with problems involving a
variety of concrete variables in standardized situations.
OTHER
SKILLS and ABILITIES:
Ability to develop effective working
relationships with students, parents, staff and the school community. Ability
to communicate clearly and concisely. Ability
to perform duties with awareness of all district requirements and the Board of
Education policies.

While performing the duties of this job, the
employee is regularly required to sit and talk or hear. The employee is occasionally required to walk
and reach with hands and arms. The
employee must occasionally lift and/or move up to 10 pounds. The employee must
be able to drive to different locations within the district on a daily basis
and occasionally out of the district. Specific vision abilities required by
this job include close vision, distance vision and peripheral vision. Employee
must be able to distinguish the difference between bells, buzzers, beeps, etc.
